Report an Incident

In the event that a student witnesses or is a victim of any crime, violence, sexual assault, building entry, damage to the building, vandalism, or other criminal activity, the student should:

  1. Retreat to safety
  2. Seek medical attention if needed
  3. Inform law enforcement of the incident (CCMS strongly suggests that a victim of sexual assault report the incident in a timely manner. Time is a critical factor for evidence collection and preservation).
  4. Complete an Incident Report.

Use the form below to report a crime, Title IX violation, or other incident. We will review and contact you shortly.  If you have been the victim of a crime, CCMS strongly encourages you to file a police report.  Springfield Township Police Department’s phone number is 513-729-1300. If you have been the victim of a sexual or interpersonal offense, you are encouraged to seek immediate medical assistance. If this is an emergency, call 9-1-1.
